Where to Apply

Applications can be picked up at your local commissary and are also available as a PDF file (click here to download). Applications may be filled out using Adobe Acrobat, however once completed, you must print and hand-carry or mail it to your local commissary. Applications may not be emailed or faxed. Individuals may apply at only one commissary. Typically this will be the commissary where his/her family does their shopping.

It is your responsibility to ensure that your complete application package is received at a commissary by close of business on the application's due date (February 22, 2013).
